VO₂ Max: The Longevity Test You Can Actually Train For

If you could track just one number to predict how long and how well you’ll live, a strong candidate is VO₂ max — the maximum amount of oxygen your body can use during hard exercise. It’s a direct measure of cardiorespiratory fitness, and the data behind it is striking.

Why it matters this much

In one of the largest analyses ever done — over 750,000 adults — each one-step improvement in fitness (≈3.5 ml/kg/min of VO₂ max) was linked to a 13–15% lower risk of death, independent of age, sex, BMI, or existing disease (JACC, Kokkinos et al.). People in the lowest fitness group carry a 2–5× higher risk of early death than the fittest — a gap that rivals or exceeds smoking and diabetes (PubMed).

The encouraging part: unlike your genes, VO₂ max is trainable at any age.

How to know your number

  • Lab test (~$100–250): a graded treadmill/bike test with a mask — the gold standard.
  • Wearable estimate (free): most modern smartwatches estimate VO₂ max from heart-rate data. Less precise, but useful for tracking trend.
  • Field test (free): a 1.5-mile timed run or 12-minute Cooper test gives a rough estimate.

How to train it

You build VO₂ max with a mix of easy volume and hard intervals:

  • Zone 2 base (most of your training): 2–4 sessions/week of 30–45 min at a pace where you can hold a conversation.
  • Intervals (1–2×/week): the classic “4×4” — four 4-minute hard efforts, each followed by 3 minutes easy.
  • Progress gradually and clear high-intensity work with your clinician if you have heart disease or risk factors.

Start this week

  1. Check your watch’s VO₂ max estimate (or do a Cooper test) to set a baseline.
  2. Add three Zone 2 sessions of 30 minutes.
  3. Once that feels routine, add one interval session weekly.
  4. Re-check in 8–12 weeks.
