Acceptance of These Terms
By accessing or using the Pierre Health website and patient portal (the “Services”), you agree to these Terms of Service. If you do not agree, please do not use the Services.
About the Services
Pierre Health provides telehealth services delivered by Isaac Pierre, PA-C, a board-certified Physician Assistant licensed in Minnesota and Wisconsin. Services may include virtual visits, secure messaging, lab ordering and review, and personalized care planning across primary care, longevity, functional, metabolic, and hormonal medicine.
Not for Emergencies
The Services are not for medical emergencies. If you are experiencing a medical emergency — such as chest pain, difficulty breathing, stroke symptoms, or thoughts of self-harm — call 911 or go to the nearest emergency room immediately. Do not use the portal or messaging for urgent or emergency needs.
Eligibility and Location
To receive care, you must be an adult physically located in Minnesota or Wisconsin at the time of your visit, consistent with the clinician’s licensure. You agree to provide accurate location and identity information.
Establishing Care
Use of the website or portal does not by itself create a provider–patient relationship. A relationship is established only when care is formally accepted and a visit is conducted.
Your Responsibilities
You agree to provide accurate, complete, and current information about your health, medications, allergies, and history, and to keep your account credentials secure. Clinical recommendations depend on the accuracy of what you share.
Telehealth Consent and Limitations
Telehealth has benefits and limitations. Some conditions may require an in-person evaluation, testing, or referral that the Services cannot provide. The clinician may determine that an in-person visit or escalation is necessary for safe care.
Prescriptions
Prescriptions are issued solely at the clinician’s professional discretion when clinically appropriate. No prescription is guaranteed. Certain medications, including controlled substances, may not be prescribed via telehealth and are subject to applicable law.
Payment and Fees
You agree to pay the fees shown for the Services you select. Fees are due as described at the time of booking or as otherwise communicated. Unless required by law, fees are non-refundable once a visit or service has been rendered.
Privacy
Your use of the Services is also governed by our Privacy Policy, which explains how we handle your information, including Protected Health Information under HIPAA.
Intellectual Property
All content and materials provided through the Services are owned by Pierre Health or its licensors and may not be copied or reused without permission, except for your own personal health records.
Disclaimers
Educational content provided through the Services is for general information only and is not a substitute for individualized medical advice. The Services are provided “as is” without warranties of any kind to the fullest extent permitted by law.
Limitation of Liability
To the fullest extent permitted by law, Pierre Health will not be liable for indirect, incidental, or consequential damages arising from your use of the website or portal. Nothing in these Terms limits liability that cannot be limited under applicable law, including the clinician’s professional responsibilities.
Governing Law
These Terms are governed by the laws of the State of Minnesota, without regard to conflict-of-laws principles.
Changes to These Terms
We may update these Terms from time to time. Material changes will be reflected by updating the effective date above.
